Please note that you only must fill out the columns 1, 2, 3, 4 and 8 of part I: "Identification of Beneficial Owner" by following the instructions below:

- Number 2: Please enter your country of citizenship
- Number 3: Please enter your address in your home country
- Number 4: Please enter your address in Germany
- Number 8: Please enter your date of birth
